- Personal information: Full legal name and date of birth
- Contact information: Home address, phone number, and personal email address (avoid using a school or parent email)

- Educational history: High school name, graduation date or GED completion, and any colleges you’ve attended
- Citizen status: Citizenship or immigration status
- Major or area of interest: Choose from JCCC’s program options
