Upon being hired, a new employee may be required to complete at least the following enrollment forms provided through the Business Office and Office of Human Resources:

  • Authorizations for approved voluntary payroll deductions.
  • Enrollments for college-provided employee benefit programs.
  • Oath of Office (as required by law).
  • Personal data sheet including but not necessarily limited to residence, address and telephone number.
  • Withholding forms for taxes.
  • INS Form 9 (as required by law)

From time to time an employee may be required to update the above.
