Consistent with guidance from the Centers for Disease Control,1 all JCCC students and College personnel must complete the following COVID-19 wellness self-check PRIOR to coming to any JCCC location each day.

Wellness self-check questions

Please review and answer each question YES or NO. You do not need to consider any chronic conditions you may have.

  1. Have you been told to quarantine/isolate by a medical provider or the health department?

  2.  In the last 14 days, have you had face-to-face contact for 10 minutes or more with someone who has or is suspected of having COVID-19?

  3. In the last 14 days, have you traveled to a location with a Centers’ for Disease Control and Prevention (CDC) or Kansas COVID-19 travel warning?

  4. Are you experiencing a new cough, shortness of breath or difficulty breathing?

  5. In the last 48 hours, have you had at least two of the following new symptoms:
    • Fever of 100.4 or above
    • Chills
    • Repeated shaking with chills
    • Muscle pain
    • Headache
    • Sore throat
    • Loss of sense of taste or smell

  6. Was your temperature 100.4 or above this morning?

If you answer YES to any question

After completing your wellness self-check share any "YES" results with your instructor or supervisor through your preferred method of communication (email, text or phone call) and await instructions BEFORE you come to any JCCC location.

If you’re off campus

  1. Stay home; do not come to JCCC
  2. Contact your supervisor or professor to report your assessment
  3. Follow Return to Campus Guidelines for staying home when ill
  4. You are strongly encouraged to contact a healthcare provider to evaluate your situation
  5. If you have reported symptoms, please answer your phone when called to facilitate contact tracing process

If you’re on campus

  1. Wear a facial covering as you leave campus
  2. Contact your supervisor or professor to report your assessment
  3. Go home and follow Return to Campus Guidelines for staying home when ill
  4. You are strongly encouraged to contact a healthcare provider to evaluate your situation
  5. If you have reported symptoms, please answer your phone when called to facilitate contact tracing process

To respect your privacy, answers to specific questions will not be collected.

Instructors and supervisors should instruct any individual who answers "YES" to one or more of the wellness self-check questions to stay home and not come to any JCCC location.

When to return

Individuals should not physically return to JCCC until:

  • they are symptom- or fever-free for 72 hours without the use of fever-reducing medication, OR
  • they have been officially released from self-isolation or quarantine.
