I. |

II. |
Residency |
| A. | Adults who enroll at JCCC who have not been residents of the State of Kansas for six (6) months prior to enrollment for any term or session are considered non-residents and must pay the out-of-state or foreign tuition. Check with the Admissions Office for complete information about residency. |
| B. | Students admitted through the Metropolitan Community College cooperative agreement will not be charged out-of-state tuition. These students will pay tuition through the Metropolitan Community College system, according to the MCCC requirements. *See note regarding financial aid. |

III. |
High School/GED Transcript. An official high school transcript, including final grades and graduation date, or the results of the GED exam must be submitted. Hand-carried or faxed copies will not be accepted. If you have completed 15 or more hours of college credit, a high school transcript or GED is not needed. |

IV. |
College Transcript(s). If JCCC is the only college you have attended, you do not need to do anything regarding college transcripts. A copy of your JCCC coursework will be available for your file. If you have taken any college level courses at another institution: |
| A. | Fill out and return the Transfer Transcript Evaluation Request Form which is page 3 of the respiratory care program application. Do not wait to submit this with the program application. To facilitate the review process we recommend you submit the request form no later than September 1. |
| B. | Contact each institution you have attended and request an official transcript be sent directly to the JCCC Admissions office, 12345 College Blvd., Overland Park, KS 66210. Each applicant is responsible for notifying each institution to have the official transcripts forwarded to JCCC. Hand-carried or faxed copies are not "official" and will not be accepted. If you have not attended JCCC within the past year, you must re-request transcripts from each previously attended school. |
| C. | If you have a current JCCC College Application on file and your other institution transcript(s) have already been received, filling out the Transfer Transcript Evaluation Request Form will trigger an early evaluation of your transcript(s) to compare to the program requirements. |
| D. | Once the evaluation has been completed, you will receive a postcard in the mail. Please contact the program office or arrange a meeting with a JCCC counselor in the Student Center, 2nd floor, if you need an explanation of the completed transcript evaluation or have questions regarding the transfer of courses. |

V. |
Disclosure Background Check Form (Required). The Disclosure Background Check Form is page 4 of the respiratory care program application. | |
It is the applicant's responsibility to make sure all application materials are received in the Admissions Office on or before October 15. Application materials received after this date may not be considered until after February 15 for any remaining class positions. |
